We’re looking for an experienced Senior Case Manager to join our Community Services team, supporting young people and their families through complex, real-life challenges.

Role Type:
Part Time Maternity Leave Cover until 9 April 2027 | 45.6 hours Fortnight  
Remuneration Range: $46.77 to $50.33 ph + Super + Salary Packaging+ Employee discounts

At Lutheran Services, our Bridges Reconnect team provide vital support to young people aged 12–18 who are homeless or at risk of homelessness. Our team works alongside young people and their families to stabilise living situations and build stronger futures through counselling, group work, mediation, and practical support.

About The Role:
As a Senior Case Manager, you will deliver high-quality, evidence-based case management to young people and their families within a therapeutic, strengths-based framework. You’ll play a key role in assessing needs, developing tailored support plans, and coordinating services that help young people achieve stability, safety, and positive long-term outcomes.

What You’ll Be Doing:
  • Deliver comprehensive case management, counselling, and advocacy for young people and their families.
  • Conduct risk assessments, intake, and ongoing support planning.
  • Facilitate mediation and family engagement to improve relationships and housing stability.
  • Provide therapeutic interventions including group work, psychoeducation, and life skills development.
  • Collaborate with key stakeholders including Child Safety, Youth Justice, Education, and community services.
  • Maintain accurate case notes, reporting, and data collection in line with compliance requirements.
  • Contribute to service improvement, program delivery, and community engagement.

What You’ll Bring:
You are a passionate and experienced case manager who thrives working with young people in complex situations.

  • A Bachelor’s degree in Human Services or related field.
  • Demonstrated experience in complex case management and therapeutic interventions.
  • Strong understanding of youth homelessness, family dynamics, and risk assessment
  • Excellent communication and stakeholder engagement skills.
  • Ability to work collaboratively in a fast-paced, client-centred environment

Mandatory Requirements:

  • Valid NDIS Worker Screening Clearance.
  • Current Blue Card (Working with Children Check).
  • Current QLD Driver’s Licence.
