Support Worker – Mental Health and Disabilities!
Location: Graceville Centre at Nambour, QLD
Pay Scale: $35.27 – $38.48 ph Base Rate + Penalties
Part-Time Position (minimum 47 hours per fortnight)
Rostered Hours / Shifts:
Wednesday & Friday | 4:30 pm – 10:00 pm (5 hours) + sleepover
Thursday & Saturday | 6:00 am – 9:00 am (3 hours)
Sunday | 8:45 am – 4:45 pm (7.5 hours)
About The Role:
Located in Nambour on the Sunshine Coast, our Graceville Centre provides support to individuals living with a mental illness and/or disability. All Graceville services are conducted by working with individuals in a strength-based and recovery focused framework. Graceville Centre operates seven programs that provide support to clients either on-site or in their own homes. We are a growing service and regularly have internal opportunities for career growth.
As a Support Worker, you will work within the Personal Development and Housing Program (PDH) with individuals, helping them to increase their independence, maintain their wellness and ultimately improve the quality of their life.
Responsibilities Will Include:
- Provide flexible, support tailored to client needs and preferences.
- Assist with personal care and daily living tasks, ensuring dignity and independence.
- Encourage community participation and social engagement for individuals.
- Support skill development and independence-building activities.
- Work collaboratively with case managers and other stakeholders.
- Maintain accurate documentation of client care and interactions.
What You Bring to the Team:
- Experience in the disability and/or mental health sector
- Relevant qualifications or be currently studying in a related field
- A strong passion for empowering individuals with a mental illness and/or disabilities.
- Excellent interpersonal skills to build positive relationships with clients and their families.
- A valid Queensland Driver’s Licence.
- A valid NDIS Worker Screening Clearance & Blue Card.
