Graceville Centre at Nambour is an integral part of the healthcare community on the Sunshine Coast. We provide tailored support for people to reimagine their lives by setting and achieving their goals and learning the skills to increase their independence.

Your Role – Supporting & Encouraging Growth:

At Lutheran Services, we believe in empowering people with disabilities to lead independent, connected, and fulfilling lives. As a Support Worker, you will play a key role in assisting individuals with personal care, social engagement, and life skills development.

As a Support Worker, you will:

  • Provide one-on-one and group support to help individuals participate in community and social activities.
  • Assist with personal care and daily living tasks, ensuring dignity and respect.
  • Support individuals in developing life skills, independence, and confidence.
  • Deliver evidence-based, person-centered care tailored to each client’s needs.
  • Promote positive behaviors and build strong relationships with clients and their families.
  • Accurately document support activities and collaborate with case managers and other stakeholders.

What You Bring to the Team:

  • Certificate III or higher in Individual Support, Disability Support, Aged Care, or Home Care.
  • A strong passion for empowering individuals with disabilities.
  • Excellent interpersonal skills to build positive relationships with clients and their families.
  • Flexibility to work within a structured fortnightly schedule.
  • A valid Queensland Driver’s Licence.
  • A valid NDIS Worker Screening Clearance & Blue Card.
