We’re looking for an experienced and values-driven Care Manager (Clinical) to step into a pivotal leadership role and help shape the next chapter of care delivery at our Immanuel Gardens Aged Care site.

The Opportunity:
As Care Manager, you’ll provide professional leadership across clinical care, compliance, and team development, ensuring residents receive safe, high-quality, and person-centred care aligned with the Aged Care Quality Standards. Working closely with the Service Manager, you’ll play a key role in driving clinical excellence, mentoring teams, and supporting the ongoing growth of the service.
Key responsibilities include:

  • Leading and mentoring clinical, lifestyle, and allied health teams
  • Overseeing care planning, medication management, and clinical governance
  • Managing admissions, discharges, and case conferences
  • Ensuring compliance with accreditation, legislation, and AN-ACC requirements
  • Supporting quality improvement, infection control, and risk management
  • Contributing to service development and continuous improvement initiatives
  • Managing clinical rosters, resources, and budgets in partnership with the Service Manager

What You’ll Bring:

  • Tertiary qualification in Nursing with current AHPRA registration
  • Demonstrated clinical leadership experience in residential aged care
  • Strong knowledge of Aged Care Quality Standards, accreditation, and compliance
  • Experience leading teams, managing performance, and supporting staff development
  • Confidence in care planning, funding processes, and clinical documentation
  • A collaborative, person-centred leadership style aligned with values-based care
