We’re looking for an experienced and values-driven Care Manager (Clinical) to step into a pivotal leadership role and help shape the next chapter of care delivery at our Immanuel Gardens Aged Care site.

The Opportunity:
As Care Manager, you’ll provide professional leadership across clinical care, compliance, and team development, ensuring residents receive safe, high-quality, and person-centred care aligned with the Aged Care Quality Standards. Working closely with the Service Manager, you’ll play a key role in driving clinical excellence, mentoring teams, and supporting the ongoing growth of the service.
Key responsibilities include:
- Leading and mentoring clinical, lifestyle, and allied health teams
- Overseeing care planning, medication management, and clinical governance
- Managing admissions, discharges, and case conferences
- Ensuring compliance with accreditation, legislation, and AN-ACC requirements
- Supporting quality improvement, infection control, and risk management
- Contributing to service development and continuous improvement initiatives
- Managing clinical rosters, resources, and budgets in partnership with the Service Manager
What You’ll Bring:
- Tertiary qualification in Nursing with current AHPRA registration
- Demonstrated clinical leadership experience in residential aged care
- Strong knowledge of Aged Care Quality Standards, accreditation, and compliance
- Experience leading teams, managing performance, and supporting staff development
- Confidence in care planning, funding processes, and clinical documentation
- A collaborative, person-centred leadership style aligned with values-based care
