Clinical Care Partner – Home Care 

Location: Sunshine Coast (working across Buderim, Gympie & Kingaroy)
Employment Type: Full Time, Permanent
Remuneration: $50.58 – $52.83/hr + Super + Salary Packaging

Join Lutheran Services and help older people live safely and independently in their own homes. As a Clinical Care Partner, you’ll provide clinical oversight and mentor Care Partners to deliver high-quality, person-centred care.

This is a role-based working across multi-sites on the Sunshine Coast, supporting multiple sites both face-to-face and online.

What you’ll do:

•    Provide clinical oversight and case management support 
•    Mentor and guide Care Partners 
•    Review care plans to align with client goals and needs 
•    Support clients, families, and health professionals 
•    Manage clinical risks and complex care needs 
•    Ensure compliance with aged care standards 

What you’ll bring:

•    Tertiary qualification in Nursing, Allied Health or a related clinical field
•    AHPRA registration  
•    Experience in aged care, community or complex care 
•    Proficiency in conducting complex clinical assessments, care planning, implementation, evaluation, documentation and budget management in line with Quality of Care principles
•    Strong understanding of risk management practices, including clinical risk assessment and mitigation
•    Strong communication and mentoring skills 
•    A full Australian driver’s licence and a roadworthy, reliable vehicle 

Desirable: Postgrad qualifications or experience in chronic disease, wound or palliative care
