Make a Real Difference – Deliver Care at Home:
  • Deliver person-centred nursing care in clients’ homes.
  • Conduct nursing assessments, develop nursing care plans, and complete clinical documentation.
  • Mentor and support Home Care Workers in the community.
  • Provide clinical advice to clients and families, ensuring dignity, choice, and independence.
  • Collaborate with Home Care Managers, Coordinators, and external health professionals.
  • Ensure compliance with Aged Care Quality Standards, legislation, and Lutheran Services policies.
About You:
  • Current AHPRA registration (Nursing).
  • Experience in home care & aged care or community nursing.
  • Strong clinical assessment and care planning skills.
  • The ability to mentor and support care staff.
  • Current Driver’s Licence and reliable vehicle.
  • Current NDIS Worker Screening or National Police Check.
