Reconnect Case Manager – Intercept Community Services | Support Young People to Stay Connected, Safe and On Track

Join Intercept Community Services as a Reconnect Case Manager, where you’ll provide early intervention support to young people aged 12–18 who are at risk of homelessness or family breakdown. This is meaningful work that helps young people stabilise their living situation, stay engaged in education, and build stronger family and community connections.

Full Time Role 
Location: Caboolture - QLD
Remuneration: $45.47 - $48.93 ph + Super + Salary Packaging+ Access to staff discounts & benefits  

About the Reconnect Program:
The program supports both the young person and their family or support network, helping them navigate difficult circumstances and build sustainable solutions. Reconnect offers counselling, mediation, and practical support to young people who are:

  • at risk of becoming homeless
  • couch surfing
  • experiencing family conflict
  • disengaging from school
  • in need of support to access training, employment or accommodation

What You’ll Be Doing:

  • Provide case management, counselling and mediation to young people and families
  • Conduct holistic assessments and create client-led support plans
  • Deliver evidence-based therapeutic interventions
  • Assist young people to access accommodation, education, training and community supports
  • Facilitate group work, life skills programs and psychoeducation
  • Work flexibly, including outreach in the community and home settings
  • Develop strong connections with local schools, services, and referral networks
  • Maintain accurate case notes, data entry, and reporting requirements

What You Bring:

  • Experience supporting young people and/or families through case management
  • Strong knowledge of youth development, family systems, risk factors and intervention models
  • High-level communication, engagement and de-escalation skills
  • Ability to work autonomously and collaboratively within a multidisciplinary team
  • Confidence using Microsoft Office and case management systems
  • Essential: NDIS Worker Screening | Blue Card (Working with Children Check) | QLD Driver’s Licence
  • Desirable: Human Services, Social Work, Psychology or related qualification
