Senior Case Manager

Full-Time, Maximum Term Position (until 25 July 2026)  
Location : Caboolture – Intercept Community Services
Remuneration Range: $45.47 to $48.93 per hour + Super + Salary Packaging+ Employee discounts

We’re looking for a Senior Case Manager to provide high-quality case management, therapeutic interventions, and advocacy to support young people navigating life’s challenges.

  • Deliver evidence-based support and interventions for young people and families
  • Conduct holistic assessments and develop individualised support plans
  • Collaborate with schools, youth justice, health, and community networks
  • Lead counselling, mediation, and group sessions to build resilience and stability
  • Work within a strengths-based and trauma-informed framework
Who We’re Looking For:
You’re an empathetic and skilled practitioner who can think critically and act with compassion. You’ll bring:
  • A degree in Human Services, Social Work, Psychology, or related discipline
  • Demonstrated experience providing case management and therapeutic support to young people and families
  • A strong understanding of child protection, youth justice, and family systems
  • Exceptional communication and relationship-building skills
  • A current Blue Card, NDIS Worker Screening, and QLD Driver’s Licence
