We’re looking for an experienced Senior Case Manager (Reconnect) to join our Community Services team, supporting young people and their families through complex, real-life challenges.

Role Type:
Part Time Maximum Term role until 30 June 2027 | 45 hours Fortnight  
Remuneration Range: $46.77 to $50.33 ph + Super + Salary Packaging+ Employee discounts

At Lutheran Services, our Intercept Community Services team supports young people and families to build stronger, safer futures. The Reconnect Program helps young people aged 12–18 who are at risk of homelessness, offering counselling, mediation, and case management to strengthen relationships, stay engaged in school or training, and access safe accommodation.

What You’ll Be Doing:
In this role, you’ll deliver therapeutic, strengths-based case management, helping young people stabilise, build resilience, and achieve positive outcomes.

  • Delivering end-to-end case management for young people and families
  • Conducting risk and needs assessments and developing tailored support plans
  • Providing counselling, mediation, advocacy, and family support
  • Supporting home stability, life skills development, and transitions
  • Working collaboratively with Child Safety, Youth Justice, Health, Education and other stakeholders
  • Facilitating individual and group programs
  • Maintaining accurate case notes, reporting, and data collection
  • Participating in community networks and service development initiatives

What You’ll Bring:

  • Degree in Human Services, Social Work, Psychology or related field
  • Strong experience in case management within youth, family or community services
  • Ability to manage complex cases using therapeutic and evidence-based approaches
  • Confidence working with multiple stakeholders and high-risk situations
  • Excellent communication, documentation, and organisational skills
  • Current Blue Card, NDIS Worker Screening, and QLD Driver’s Licence
