Part of Lutheran Services, the team at Intercept support young people get back on their feet; Education is a foundation that can help. The YJET Program supports young people aged 14-16 improve their literacy and numeracy in a flexible, supportive, and youth-friendly environment, with a focus on becoming job-ready or reintegrating back into mainstream schooling.


About The Role:

As the Transitional Senior Case Manager, you will:

  • Support young people engaged in the YJET program to transition into further education/training or into employment 
  • Develop interventions including increasing life skills, goal setting, advocacy, referrals to external services, capacity building and group work 
  • Use your skills to build and maintain positive relationships with the young people you work with
  • Work within the YJET classroom, as well as in outreach within the community

** This is a Part Time Fixed Term role, ending 30/06/2025 **

About You:

Use your knowledge and demonstrated expertise in this field to ensure holistic support for clients in a challenging environment.

  • Bachelor of Human Services or related degree level qualification in human services delivery and data management systems
  • Ability to provide multidisciplinary advice
  • Knowledge of statutory requirements, program and organisational policies and procedures.
  • This position requires maintenance of a positive Working with Children (Blue Card) check, NDIS Worker Screen and current Australian Driver’s License.
