We are seeking an experienced and dedicated Care Manager who is passionate about providing high-quality care to older people. The ideal candidate will have previous experience in a similar role and a strong understanding of the aged care industry and regulatory requirements. They will also possess excellent leadership and communication skills, and the ability to build effective relationships with residents, staff, and families.

About the role:

As the Care Manager, you will have a critical role in ensuring that our residents receive the highest standard of care and feel secure in their environment. Your responsibilities will include:

  • Implementing and maintaining a person-centred model of care, promoting wellness and engagement for residents
  • Ensuring compliance with legislative requirements and the Aged Care Quality Standards
  • Experience in change management and ability to participate in the on-call roster
  • Optimising AN-ACC funding
  • Providing professional leadership and management to clinical and allied health team members
  • Developing and implementing care plans and conducting assessments and clinical education | mentoring
  • Monitoring and evaluating the quality of care provided
  • Ensuring effective communication and collaboration with residents, families, and other stakeholders
  • Contributing to the development and implementation of policies and procedures

About you:

The ideal candidate will have:

  • Previous experience in a similar role, preferably within the aged care industry
  • Committment to clinical education and mentoring to staff
  • A strong understanding of the Aged Care Quality Standards and regulatory requirements
  • Excellent leadership and communication skills
  • The ability to build effective relationships with residents, staff, and families
  • A passion for providing high-quality care and improving the lives of older people
  • Strong organisational and problem-solving skills
  • Relevant qualifications in nursing or a related field
  • A current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
