Support at Home is about putting bringing joy to clients as a Clinical Care Partner with Orana Home Care team. You will provide case management oversight and mentorship to our Care Partners, ensuring older people with complex care needs receive safe, responsive, and person-centred support in their homes and communities.

Location: Orana Home Care - Kingaroy. 
Employment Type: Full Time Permanent
Remuneration: $50.58 – $52.83 ph + Super + Salary Packaging+ Employee Discounts & Benefits

About The Role:
As a Clinical Care Partner, you will drive our values  of innovation, integrity, kindness and empowerment and;

  • Provide case management oversight, mentoring, and guidance to Care Partners.
  • Review assessments, care plans, and ensure services align with client goals and preferences.
  • Champion person-centred, trauma-informed care that respects dignity, rights, and culture.
  • Partner with clients, families, and health professionals to improve care outcomes.
  • Monitor risks, manage transitions of care, and resolve clinical concerns.
  • Ensure compliance with aged care legislation, quality standards, and organisational policies.
  • Contribute to audits, quality reviews, and continuous improvement activities.

About You:
We’re looking for someone who brings:

  • Current AHPRA registration in Nursing, Allied Health, or a related clinical field.
  • Experience in complex case management, ideally in community, aged care, or complex care.
  • Strong knowledge of aged care legislation, standards, and clinical governance.
  • Excellent communication and mentoring skills to guide staff and support clients.
  • A valid QLD Driver’s Licence, NDIS Worker Screening or police check, and reliable vehicle.
    Desirable: Postgraduate qualifications in case management, chronic disease, wound management, or palliative care
