About the Role:
This is a rewarding opportunity for a professional who is passionate about aged care and thrives in a role that blends client engagement, planning, and operational coordination.
As a Home Care Coordinator, you’ll play a key role in:

  • Supporting client independence through quality care planning, goal setting and budget management
  • Coordinating and delivering services that align with aged care standards and individual client needs
  • Maintaining positive client relationships and liaising with carers, family, and external service providers
  • Ensuring compliance, continuous improvement, and service excellence across sites
  • Collaborating with team members to deliver safe, person-centred home care
  • Participating in onboarding, offboarding, and ongoing assessment processes
  • Supporting client visits across all locations

About You:

You’ve got strong administration and coordination skills. You thrive in an environment where high performance meets genuine care and kindness. You share our values of Innovation, Integrity, Empowerment and Kindness and bring:

  • Experience in Home and Community Care, Aged Care, or Case Management
  • A strong understanding of aged care compliance and quality service delivery
  • Excellent communication, planning and coordination skills
  • A client-first mindset and commitment to supporting older people to live independently
  • A current QLD Driver’s Licence (essential)
  • A Police Check and NDIS Worker Screening (or willingness to obtain)

Desirable:

  • A degree in community or aged care-related field
  • Experience with rostering systems and budget management
  • Strong computer literacy and community networks
