The Home Care team at Lutheran Services are seeking a dedicated Registered Nurse to their team servicing older people in the Woodridge area.  Home care clients will be in the following areas: Woodridge, Logan and clinical visits to clients across Nundah and Tallebudgera too. You could work a day at each site?

About the role:

Promote and maintain the health and wellbeing of clients in the comfort of their own homes  

Guide and support client independence, empowering choices, enhancing overall quality of life.  

Build strong and trusting relationships, with clients, families and across the support team to ensure that our clients receive the highest standard of physical, emotional, and social care, tailored to their specific needs.    

This is a part time role, 45 hours | 6 days per fortnight with a view of transitioning to a full-time position.

  • Conducting nursing assessments and providing clinical care to home care clients
  •  Supporting and mentoring home care workers in the community with education
  • Provide clinical nursing advice and support to our client and their families
  • Liaise closely with our Home Support Manager and Home Care Co-ordinator
  • Collect and report information and complete assessments to assist with the formation and review of nursing care plans and funding submissions for each client
  • Establish and maintain collaborative relationships within the service and also members of the external health care team.

 

About you:   

Your compassion and commitment to providing respectful and effective care to our older clients will be greatly appreciated.   A current AHPRA registration is required and either a company care or mileage allowance paid if using your own vehicle.
