Join Salem Home Care team based at Toowoomba for a contract role until Jan 2027 with the review to extend/go permanent. Focused on serving those who matter most to us - our clients - by delivering compassionate, person-centred care in their homes.

Employment Type: Contract (Mat Leave)
Remuneration: $39.96 - $48.43 p/h (experience dependent) + Super + Salary Packaging + Staff discounts & benefits.

Make a Real Difference – Deliver Care at Home:
  • Deliver person-centred nursing care in clients’ homes.
  • Conduct nursing assessments, develop nursing care plans, and complete clinical documentation.
  • Provide clinical advice to clients and families, ensuring dignity, choice, and independence.
  • Collaborate with Home Care Managers, Coordinators, and external health professionals.
  • Ensure compliance within Aged Care Quality Standards, legislation, and Lutheran Services policies.

About You:

  • Current AHPRA registration (Nursing).
  • Strong clinical assessment and care planning skills.
  • Ideally experienced in home care & aged care or community nursing.
  • Current Full Australian Driver’s Licence and a fully comprehensively insured and reliable vehicle. Must be willing to drive between locations where required to do so.
  • Ability to support care partners and support staff with clinical needs.
  • Current National Police Check (or willing to obtain).
