Support at Home Transformation Program Manager 

We are looking for a Support at Home – Business Transformation Manager who can grow our Home Care Program and set us apart from the competition as a provider that makes your heart smile." You will be based at our Support Center in Milton in a full-time, fixed-term position for 18 months.

About the role:  

The Support at Home Transformation Program Manager is a fixed-term position for 18 months. This role will lead and oversee the implementation of strategic initiatives to ensure readiness for delivering a scalable, competitive, financially viable, and customer-focused Home Care service. These efforts align with the Support at Home legislative changes effective June 1, 2025.

Key Accountabilities:  

  • Lead the transformation of our Home Care program in line our Lutheran Services’ Model of Care and Service, Lutheran identity and new legislative changes. 
  • Develop and execute a comprehensive transformation program that integrates multiple initiatives and fosters effective collaboration across departments.
  • Create and execute a comprehensive transformation project plan for Support at Home services post-July 2025.
  • Strategically integrate program initiatives to support the General Manager of Home Care in stabilising the current Home Care business throughout the transformation process.
  • Create and maintain a program budget, conduct risk assessments, and develop mitigation strategies. 

Essential Criteria:   

  • Demonstrated experience in overseeing business transformation and/or improvement programs in a comparable organisation. 
  • Strong understanding of home care services and health care industry trends.
  • Leadership skills with a focus on fostering teamwork, collaboration and a positive work environment. 
  • Strategic planning, budget development and resource management skills to plan, track and report on program success. 
  • A desire to grow a program that stands apart for its customer-focussed care.
