Remuneration: $140K - $145K + Superannuation + Salary Packaging + EAP and other Benefits
If you’re a passionate Nurse Practitioner this is an opportunity to thrive in a role where advanced clinical practice meets meaningful impact. At Lutheran Services, you’ll work autonomously while being fully supported by a collaborative, multidisciplinary team. You'll join an establised group of five Nurse Practitioners, complemented by our recently recruited Diabetes Educator. Your expertise will directly enhance residents' wellbeing, strengthen clinical decision‑making, and deliver high‑quality outcomes across two respected aged care communities.
About You
You’re clinically confident, values‑led, and committed to improving the health and quality of life of older people. You bring advanced assessment skills, strong problem‑solving capability, and a partnership-focused mindset. Ideally, you have:
- Endorsed Nurse Practitioner with unrestricted AHPRA registration
- Experience in aged care, chronic disease, dementia, palliative care or complex co‑morbidities
- Capability to prescribe, order diagnostics, and deliver autonomous clinical care
- Advanced communication and relationship‑building skills
- Strong clinical leadership capability with the ability to mentor and support teams
- Ability to work both independently and collaboratively across sites
Desirable: Experience across geographically diverse locations or a passion for mental health and palliative care
About the Role
This role provides advanced, autonomous clinical care to residents while supporting GPs and site teams to achieve better outcomes, reduce avoidable hospital transfers and build clinical capability across the sites. You align to our values of kindness, integrity, empowerment and innovation and will:
- Deliver advanced assessment, diagnosis, prescribing and care planning for complex conditions
- Strengthen collaborative working relationships with GPs and specialists
- Prepare and streamline processes to maximise GP on‑site time and efficiency
- Reduce unnecessary hospital transfers through timely interventions and clinical management
- Support medication reviews, reduce polypharmacy and introduce non‑pharmacological strategies
- Provide expert advice in palliative care, dementia support, wound care and end‑of‑life planning
- Build clinical skill capacity of nursing teams through coaching and knowledge‑sharing
- Work within the Nurse Practitioner Scope of Practice to ensure safe, high‑quality care
- Provide direct clinical care as needed across both sites
