Care Partner – Home Care
Location: Tabeel – Laidley, QLD
Hours: 30 hours p/week, Permanent Part Time – 4 days per week (days can be negotiated)

We are seeking a compassionate and organised Care Partner to join our Home Care team. This role is ideal for someone who thrives on building meaningful relationships and delivering truly person-centred care to older Australians living at home.
About the Role:
As a Care Partner, you will play a key role in supporting clients to live independently with dignity and choice. You will work closely with clients and their families to understand their needs, develop tailored care plans, and ensure services are delivered effectively and within budget.
This is a hands-on, relationship-focused role where you will:
- Meet with clients (initially face-to-face where possible) to understand their needs, goals and funding levels
- Develop and manage personalised care plans aligned to individual preferences and budgets
- Conduct regular client check-ins (minimum monthly contact in line with government requirements)
- Review and adjust care plans to ensure they remain relevant and effective
- Liaise with families, carers, and service providers to coordinate supports
- Monitor budgets and ensure services are delivered within allocated funding
- Maintain accurate documentation and ensure compliance with aged care standards
Where face-to-face visits are not practical, ongoing engagement may occur via video calls.
About You:
You are someone who genuinely cares about people and enjoys building trust and connection. You are also organised, proactive, and confident managing multiple clients and priorities.
You will bring:
- Experience in aged care, home care, or case management
- Strong understanding of Home Care Packages, Support at Home, or similar funding models
- Demonstrated experience in care planning, assessments, and budget management
- Excellent communication skills and ability to engage with clients, families, and stakeholders
- A person-centred, culturally safe and respectful approach
- Strong organisational and documentation skills
- Ability to work both independently and collaboratively
Requirements:
- Current Driver’s Licence and access to a vehicle
- National Police Check (or willingness to obtain)
