Join Lutheran Services, Trinder Park Home Care tean as Clinical Care Partner and help older people live safely and independently in their own homes. As a Clinical Care Partner, you’ll provide clinical oversight and mentor Care Partners to deliver high-quality, person-centered care.

Location:  Working primarily across Woodridge and Toowoomba with some travel to satellite sites with mileage reimbursed.
Employment Type: Permanent Full Time

Remuneration: $53.63 - $54.74 (based on Experince & with Statement of Service) + Super + Salary Packaging + Access to Staff Discounts & Benefits 

What You’ll Do:
In this role you will support multiple sites both face-to-face and online, this is not a remote role, it will be based at our Woodridge site. Travel to other sites will be arranged between successful candidate and service, using a corporate vehicle or your own vehicle (with mileage paid).  

  • Provide clinical oversight and case management support
  • Mentor and guide Care Partners
  • Review care plans to align with client goals and needs
  • Support clients, families, and health professionals
  • Manage clinical risks and complex care needs
  • Ensure compliance with aged care standards 

What You’ll Bring:

  • Tertiary qualification in Nursing, Allied Health or a related clinical field
  • AHPRA registration  
  • Experience in aged care, community or complex care
  • Proficiency in conducting complex clinical assessments, care planning, implementation, evaluation, documentation and budget management in line with the Aged Care Act 
  • Strong understanding of risk management practices, including clinical risk assessment and mitigation
  • Strong communication and mentoring skills
  • A full Australian driver’s licence and a roadworthy, reliable vehicle
