About Us: 

Part of Lutheran Services, Trinder Park home care in Woodridge, and St Andrews home care in Tallebudgera, supports those to live in their own homes for longer- to live life to the fullest with a reliable, friendly carer by their side, with whatever support they need.

About the role: 

The Home Care Coordinator:

  • Ensures that care needs are being met through high-quality service delivery
  • Supports clients in developing or reviewing their Client Goal/Care Plan
  • Coordinates and monitors the delivery of services, maintaining appropriate communication with carers and family members
  • Mentors home care workers in the community for their benefit, and that of their clients.

This is a permanent, full-time position,  3 days based at Woodridge and 2 days at Tallebudgera.

  • Can make sound judgements and develop high-level Client Goal plans based on your knowledge of Home and Community Care
  • Enjoy working with a team to deliver services
