- Deliver evidence-based care and lead a multidisciplinary team
- Conduct assessments and create tailored care plans
- Support and mentor care staff

About You:

- Experience with aged care software systems
- A passion for elder care and advocacy for person-centred clinical care
- Ability to work a rotating roster
- Strong communication and interpersonal skills
- Group facilitation and one-on-one support skills
- Minimum 12 months postgraduate experience (desirable)
- Full working rights in Australia
