Part-Time (6-Month Contract) | Monday to Wednesday | 7:30 AM – 3:30 PM
Contract Period: 07/04/2025 – 07/10/2025 (Potential for transition to part-time or full-time employment upon contract completion.)
Remuneration: $31.78 – $38.74 per hour + Salary Packaging (up to $15,990) + Access to staff discounts & benefits.

Your Role – Coordinating Care, Empowering Lives:
We are seeking a dedicated and experienced Home Care Coordinator to oversee and support the care of our residents at Zion Gympie Home Care, ensuring compliance with Lutheran Services' policies and procedures. Your role is vital in ensuring that each client receives personalized, respectful, and high-quality care that aligns with their individual goals.
- Support and guide clients in developing care plans and goals tailored to their needs.
- Monitor and manage care packages, ensuring compliance with government regulations.
- Work closely with personal care workers, families, and external providers to deliver high-quality services.
- Conduct risk assessments and coordinate third-party assistance when required.
- Maintain accurate records, optimize system tools, and oversee client onboarding and service management.
- Relevant Qualification in Nursing, Aged Care, Community Services, or a related field.
- Previous Experience in care coordination, case management, or a similar role within the home care or aged care sector.
- Strong Knowledge of the Aged Care Quality Standards, Home Care Packages (HCP), and other relevant regulations.
- Excellent Communication and Leadership Skills to effectively liaise with residents, families, and multidisciplinary teams.
- Ability to Develop and Implement Care Plans tailored to individual resident needs.
- Commitment to Person-Centered Care, ensuring the well-being and dignity of residents.
- Proficiency in Care Management Systems and Documentation to maintain accurate records.
- Current First Aid and CPR Certification (or willingness to obtain).
- Valid Police Check and NDIS Worker Screening Clearance (or eligibility to obtain).
- Current Australian Driver’s License (preferred for community-based visits).
