About the Role:
You’ll deliver high‑quality, person‑centred nursing care to clients in their own homes, supporting them to live safely and independently. Working closely with Home Care Managers, Coordinators, and community teams, you’ll play a key role in clinical leadership and care continuity. Every Wednesday your shift will overlap with our other talented RN - to ensure for a collaborative working experience. Share a handover that ensures continuity of care. Your responsibilities include:
- Delivering individualised nursing care in clients’ homes
- Conducting nursing assessments, developing care plans, and completing clinical documentation
- Providing clinical advice to clients and families with dignity, choice, and respect
- Collaborating with internal teams and external health professionals
- Ensuring compliance with Aged Care Quality Standards, legislation, and Lutheran Services policies
You’re a compassionate and capable Registered Nurse who enjoys autonomy, variety, and building strong client relationships. You’ll bring:
- Current AHPRA registration (Nursing).
- Strong clinical assessment and care planning skills.
- Ideally experienced in home care & aged care or community nursing.
- Current Full Australian Driver’s Licence and a fully comprehensively insured and reliable vehicle. Must be willing to drive between locations where required to do so.
- Ability to support care partners and support staff with clinical needs.
- Current National Police Check (or willing to obtain).
