Part of Lutheran Services, and based at Trinder Park Home Care in Woodridge, we are a leading provider of Home Care services dedicated to improving the quality of life for our clients.

About the role: 

We are looking for a dedicated and organised Client Coordinator to join our team and help us to continue providing exceptional care to our clients. You will:

  • Collaborate with Home Care teams, ACAT assessors and clients to develop connected communities
  • Support clients in developing or reviewing their goal plans to meet their needs through high quality service delivery
  • Coordinate and monitor the delivery of services, maintaining appropriate communication with carers and family members
  • Support home care workers in the community for their benefit, and that of their clients.

This is a full time contract position, starting 15 October

About you: 

If you have a passion for elder care and a strong dedication to providing customer-focused care, this role may be for you. If you:

  • Have previous experience in client-facing or administrative roles, ideally in healthcare or home care services
  • Enjoy working with a team to deliver services, focussing on customer service
