About the role:
We are looking for a dedicated and organised Client Coordinator to join our team and help us to continue providing exceptional care to our clients. You will:
- Collaborate with Home Care teams, ACAT assessors and clients to develop connected communities
- Support clients in developing or reviewing their goal plans to meet their needs through high quality service delivery
- Coordinate and monitor the delivery of services, maintaining appropriate communication with carers and family members
- Support home care workers in the community for their benefit, and that of their clients.
This is a full time contract position, starting 15 October
About you:
If you have a passion for elder care and a strong dedication to providing customer-focused care, this role may be for you. If you:
- Have previous experience in client-facing or administrative roles, ideally in healthcare or home care services
- Enjoy working with a team to deliver services, focussing on customer service
